Licensed Practical Nurse (LPN) to join our dedicated, compassionate

www.cancercareyork.com.What you will be doing...The Oral Oncology
Clinical Coordinator - LPN provides clinical coordination of care
for patients on oral therapy under the direction of the Oral
Clinical Manager. ESSENTIAL FUNCTIONS JOB DUTIES: The following
duties are considered essential to the job. Qualified individuals
must have the ability with or without accommodation to perform the
following duties:1) Assists Oral Oncology Nurse Navigator with
handling Oral Navigator referrals.2) Assists with management of
incoming oral therapy-related patient calls regarding
nursing/medical questions, medication compliance, and follows up
accordingly and documenting in the Electronic Medical Record
System3) Assists with monitoring and documenting the patients
adherence with oral therapy.4) Gathers clinical information with
incoming calls and initiates proper communication/follow-up with
MD/AP.5) Communicates with outside facilities to coordinate care.
(Radiation, skilled nursing facility, hospital)6) Confirms labs,
imaging, and follow-up appointments are scheduled appropriately in
conjunction with the date the patient started oral therapy.7)
Assists Oral Oncology Nurse Navigator or Oral Clinical Manager with
any walk-in/ sick patients visits or concerns this may include
vital signs, evaluating concern of pt and obtaining blood for lab
analysis via venipuncture as directed.8) Reviews assessment of the
patients lab work for possible intervention including dose
modifications or clinical assessment of symptoms with MD/APP.9)
Ensures proper communication with patients and families regarding
course of action, care planning as directed by MD/AP.10) Pull all
Work queue data and staff incoming messages from EPIC computer
system, organizes and triages for proper management.11) Maintains
oral therapy education materials by keeping them current and
organized and communicates with representatives regarding education
materials.12) Prepares and clinically reviews teach materials for
intervention, including drug interactions, lab/testing needed,
communication with other offices regarding management with
diabetes, HTN, etc.13) Collaborates with Medically Intergraded
Dispensary (MID) regarding patients coordination of care.14)
Maintains regular and punctual attendance.15) Comply with all
practice policies and procedures including CCAYs Code of
Conduct.SECONDARY FUNCTIONS JOB DUTIES: The following duties are
considered secondary to the essential functions listed above:1)
Acts as a back-up in MID for medication check.2) Maintains and
updates CCAY staff compliance, including training, policies and
procedures.3) Performs other duties as assigned by Oral Clinical
Manager.GENERAL FUNCTIONS OF LPN PER NURSE PRACTICE ACT PERTAINING
TO CCAY1) The LPN may accept a written order for medication and
therapeutic treatment from a practitioner authorized by law and
facility policy to issue order.2) The LPN may accept an oral order
if the oral order is authorized by a physician, advanced
practitioner by the facility policy. Oral orders include telephone
orders Oral orders must be immediately transcribed to the medical
record of the patient. This transcription must include the name of
the ordering practitioner, date, time and full signature of the LPN
accepting the oral order. The LPN is required to question any order
which is perceived as unsafe or contraindicated for the patient or
which is not clear. The LPN is to raise the issue with the ordering
practitioner first, but if unavailable, a registered nurse should
be consulted to assist the LPN.Requirements:What we look

Accredited LPN programCurrent License of a Practical Nurse by State
