Job Description:
*Bell Socialization Services* is a nonprofit organization
dedicated to empowering individuals with mental illness,
intellectual disabilities, and housing insecurity. Our team
provides compassionate care and support to help individuals live
more independent and fulfilling lives. We are seeking a dedicated
Licensed Practical Nurse (LPN) to join our team and provide
compassionate medical care to individuals in our residential and
community programs. The LPN will work closely with clients,
caregivers, and interdisciplinary teams to ensure the health,
safety, and well-being of those we serve.

*Key Responsibilities:*
--- Provide direct nursing care, including medication
administration, wound care, and monitoring vital signs.
--- Conduct health assessments and report concerns to healthcare
providers.
--- Assist with medication management, education, and compliance
for individuals in our programs.
--- Maintain accurate medical records in compliance with state and
organizational policies.
--- Educate and support staff and caregivers on health-related
concerns and best practices.
--- Collaborate with case managers, therapists, and community
resources to coordinate care.
--- Advocate for clients' health needs and ensure a safe,
supportive environment. Qualifications:
--- Active LPN License in Pennsylvania (required)
--- Driver's License (required)
--- Experience in mental health, intellectual disabilities, or
community-based care (preferred)
--- Strong communication and interpersonal skills
--- Ability to work independently and as part of a team
--- CPR & First Aid Certification (or willingness to obtain)
--- Passion for serving vulnerable populations
Job Type: Full-time
Pay: $52,520.00 per year
_*Minimum requirements*__: HS diploma/GED, current PA LPN license,
and two (2) years of professional experience; ability to complete
physical and PPD tests. Must be at least 21, possess a valid
driver's license, insurable driving record, and reliable
transportation. Current physical and Mantoux (TB) test required. PA
State Police background check, Act 33, and/or FBI clearances.
